H5P - Question Set: Multiple Choice

Hi!

Here is my question...not even sure whether its possible but here we go....haha.

I am hoping to create a 2 question H5P multiple choice set where students select one option from each question.  Then when they are done, I want their selection choice to appear in the gradebook.  

For example: 

Q1:  Which best describes your work this term?

a) I did my best

b) I tried very hard

c) I didn't try at all

Suppose the student selects c).  Then I want "I didn't try at all" to appear in grades beside the activity. 

This is at all possible?? Is there a work around or better option??
