Create new textbox with Icon and Headline

I would like to have a new template for an H5P content type.
I need a colored text box with an icon in which I can insert any image, and which is integrated into a colored headline.
How can I create an H5P content type file similar to the flashcards.h5p file, but with non-rotatable text boxes?
Is there a guide for this?
