Interactive Video: Adding a Form Interaction

Hi, Is it possible to add a form interaction on the interactive videos. For example, I want the user enter his name and email address using forms when the video has reached a specific time.

If it is not possible, Is there anyway to do this? Currently the only way I can think of is just to link it to them

This is currently not possible. I guess the name and email should be stored and displayed somewhere?
