Bugs in content types

Hi there, 

When I add a single choice question into my course presentation, I see an idicator (when hovering that says 1/2)? I can't see a place to edit this in the question/course pres settings...

I am also unsure why the question is being marked as 2/2?

could you help me with this issue:

When someone put to his site generated code of h5p content from my Drupal 8 site, after every clearing drupal cache(for examp. "drush cr" command) on clients side(where embed code is inserted) instead the content appears:
"The website encountered an unexpected error. Please try again later." - message.

Do you have any idea why I'd get an "invalid security token" (cf. screenshot) when trying to load an .mp3 into the Course Presentation tool?
