Quiz Grades Missing

Bugs in content types
  1. Detailed steps to reproduce the bug (exactly how and when did it happen): Ongoing when quiz/exam completed
  2. Platform and version number. E.g. Drupal, Wordpress, Moodle. Moodle 3.7.7

  4. Browser: Chrome, Firefox, Safari etc: Chrome
  5. H5P plugin version: mod_hvp version 1.21.0
  6. H5P content type and version (if a content type was used), and a sample URL or attached H5P.
  7. Any browser console errors n/a
  8. Any PHP errors: unsure
  9. Screenshots if it's a visual problem: n/a
  10. Any recent changes to the environment (new plugins/modules, updates, configuration changes etc.): unsure
  11. Any recent changes to the browser you're using (new extensions, updates etc.): n/a

I have been tasked with tracking down a problem so unfortunately I may not be able to provide all of the details (like recent updates) needed to fully track down the cause of this issue.

Apparentlyi recently in a course, grades for completed exams stopped posting in the gradebook.  Grades worked for exam #1 but have failed for exam #2 and #3.   I have checked in the database and find scores logged in mdl_hvp_xapi_results and entries appear in mdl_grade_grades, but starting with exam number #2 and moving forward, the rawgrade recorded in mdl_grade_grades is null. 

Does anyone have any idea why grades would start recording as null?
