Vertical spacing for fill in the blanks in a column

I have created a fill in the blank activity in a course presentation, where a student looks at a food label, then fills in blanks in a chart. Each blank is located in the same column, but on a different row. I want the blanks to align with the cells in my chart, and I only want one checkbox.

Is there a way to adjust the vertical spacing between each entry, so I can get the fill in the blank to line up with the cells of the chart?  Currently, I can get the first blank in the right spot, but the ones after that are either too high in the cell, or too low.
