Bugs in content types

I could not possible test how often this happens, but we get a couple calls about this a day. This happens with all multiple choice questions. The only other question type we have is true and false however, but they seem all to work perfectly.

1. Detailed steps to reproduce the bug (exactly how and when did it happen). Answer: Step 1) Begin playing the H5P interactive video with two summary questions. 2) Watch entire video until Summary section. 3) click Summary, answer questions - see screenshot. 4) Go to Gradebook, no values appear. 5) Check Results report for the activity, "Gradebook score: 100 out of 100" but nothing is displayed under "Your Answer" section. 2. Platform and version number. E.g.

 In moodle, the files of some libraries have disappeared, but in the database it indicates the file.

How can I upload these files again in the moodledata?

I trie to load the libraries again, with a presentation, but it dos not reload the libraries, then it still does not work.
