Does this happen on all your contents? Do you see this issue on stand alone Question Sets or does happen if it is inserted inside another content type. Can you please upload a sample content or provide a URL of the content?
The issue occurs in the 'Question Set' tool so, I updated the content type cache and library.
I find that when I access and update an individual 'Question Set', the relevant set subsequently appears updated on the webpage. Should all not be updated following the relevant cache and library update?
To me this looks as if all latest libraries have been installed, but for some reason, the old version of QuestionSet is loaded. Please try to remove the files inside /wp-content/uploads/h5p/cachedassets. These will be re-created once you visit content the next time, and they then should contain the correct version of QuestionSet.
