A free HTML5 based content type allowing creatives to create questionnaires. Questionnaires can be used as surveys or open ended questions. You may use multiple choice questions or text input questions. All you need is this content type and H5P.com or the H5P plugin in publishing systems like Canvas, Brightspace, Blackboard, Moodle and WordPress.

Description

Gain feedback and ask open ended questions in Interactive Videos and other content types with Questionnaire. Questionnaire makes the user's answers available via xAPI. This means that website owners may store the answers in many different ways. Answers may be stored in an LRS, the sites own custom storage or a script may for instance fetch the e-mail address and use it to send the user an e-mail. On H5P.org answers are stored in Google Analytics.

To retry the questionnaire just refresh the page.

In order to enable H5P Questionnaire on your site you will need to:

Enable LRS dependent content types under H5P Settings (the location of the settings will differ depending on the platform you're using)
Set up a service to process the events (typically an LRS)

Set the accessibility messages for screen readers

Cannot find the destination field to customize this questionnaire.

The answers are not stored anywhere. They are dispatched so that you can do whatever you want with them. You need some system for handling events to receive the answers and store them somewhere yourself. A typical system for this would be a LRS (Learning Record Store) that will receive the events and save them for you to see at a later time.

There should exist plenty of LRS software out there. Using an LRS will also provide several other benefits such as tracking of scores over time for the different H5Ps and seeing what different users answer. Setting up and configuring an LRS is left to the web administrator.

Restricted content type

I tried to create a questionaire type content in my wordpress. However, I can't since it says "the use of this content type has been restricted by an administrator." even though the restricted option in libraries is unchecked.

Could you tell me the way to enable it in wordpress?

Also, once a user submits an open ended question, what event will be triggered? What I want is to save the user's respons using externalDispatcher.

Your input would be appreciated.

Hi, the Questionnaire content type is a bit special, you have to go into H5P settings and check the "enable LRS dependent content types", because it does not work well without an LRS. I have attached a screenshot showing where you can find the option.

Every time a user interacts with a question a 'interacted' xAPI event is sent, and when the whole questionnaire has been answered a 'completed' event is sent. This is the common event names for all H5P libraries, though some libraries may offer more detailed events as well, such as 'progressed', 'answered', etc.

I use interactive contents "Questionairre", but one in moodle, it work good but is impossible analyse the answers of students, beacuse the contents doesn't register the answers. What I did was I just went to the site administration and and searched for "LRS", from there I enabled the LRS in Moodle. Although I believe LRS is already enabled in your system since you are able to use Questionnairre.

For the other concern about reviewing the answers to each question as Thomas suggested you have install a separate software and configure it to pick up the activities done in Questionnaire.

Not with just the H5P plugin, but yes. You can use the plugin H5PxAPIkatchu to store xAPI statements in the database of WordPress. There's a short demonstration video, too, although it's a little outdated.

If you want to send the statements to an LRS, you should use wp-h5p-xapi. It will allow you to define your LRS as the endpoint.

I would assume that you are logged in as the administrator. Having said this please check if any of libraries are restricted, to do this go to site administration -> plugins -> H5P libraries (path may differ for different versions of Moodle). If it is not restricted in the list you can download the sample above and upload it to Moodle.

"Other" (Own answer) option

Hello everyone.
I am currently working on a LMS through wordpress and I'm using H5p for interactive content. I'd like to build in surveys for feedback and other stuff.
The questionnaire seems perfect and I implemented it. Though there is on issue:

You can't have a simple Multi-choice question with a text input field for users to type in their own answers.

Example: how did you hear about us?
- Youtube
- Friends
- Ads
- Others: _______

This is a pretty important feature. Is there anyway to build that in myself or other H5P content which would better suit this situation?

I'm afraid this is not possible without making changes to the code. However there is a workaround where you can instruct them to place notes on the next page. I know this may not be perfect with your need but this content is a bit limited as you noticed.

Lastly there are no other content types that allows you to have something similar to a survey since all of them generates a score which in my own opinion is not a good thing for surveys.

Questionnaire input rows

Just been trying to create a test Questionnaire in my H5P account. I can see that you can change the input rows (1 line, 2 lines etc.). However this doesn't seem to do anything when responding to the question. It lets me go onto multiple lines.

So why have a setting to restrict lines when it doesn't work? Or am I missing something completely?

Hi Catherine,

The "lines" option does not restrict users to have answers that is more than what was set. It is there to have some sort of a guide for users as well as avoid needing to scroll for long answers. I think it was also designed this way with aesthetics in mind, if it did not have the option either you would have a space that is too small for the answer (needing to scroll) or there is too much space when only a few words are needed.

Questionnaire doesn't save the results in "My Results" tab

Hi!

I use Wordpress + LearnDash + Tin Canny LearnDash Reporting + H5P.

Unfortunately Questionnaire module doesn't save the results in "My Results" tab unlike other modules.

TinCanny integrated LRS saves in activity tables only verbs like "correct" "incorrect", "Interacted", "Attempted" etc. - but not the question answers or text from open ended questions.

And I couldn't find any option to send results to email address.

That's very sad!

Interactive Video & Questionnaire

I am new to this system and trialing out.

Is it possible to add a Questionnaire at the end of an interactive video? If so how?
Also I do not have a wordpress site, moodle or other site I am currently trialing direct with H5P.Com is it still possible to retrieve the answwrs to a questionaire that my students will be answering during this trial stage? If so can you advise what I need to do or direct me to a learning module for this.

When you say "at the end of an interactive video" if you mean after the video then the Questionnaire will appear I'm afraid this is not possible. All activities needs to be placed within the Interactive Video.

With regards to seeing the results you will need the Learner reports feature in your account. To learn more about this you can send an email to [email protected].

OK, if I place the Questionnaire in the interactive video where is the the Questionnaire template in the interactions? I can see true/false questions, but nothing that can be used to make a questionnaire. I was wanting to have my questionnaire have a likert scale.
